I have also released any medical situations needed to be known, to the instructors. Elsewise, my child is physically capable of dance, cheer or gymnastic activity. I, also agree To the monthly payments and registration fee according to the fee schedule of the class(es) my child is taking, is required for my child’s tuition during the month they are rendered. I agree to pay a $5 late fee if they are not. I also agree that payment for costumes or other fees will be due at particular times throughout the year. I will pay for any collection fees.
